- Please upload your charts in .pdf or .tif formats
- Please do not password protect your documents
- If you like to receive a confirmation email, please enter your email address below

- To get started, simply enter the following REQUIRED details:
- Letter Reference Number: This should be on the chart request letter you received
- NPI Number
- Mail Address: This is an optional field

- If the request letter is related to multiple claims, you can upload all of them or some of them. You can always come back and upload the rest later.
